Contact information for the corresponding author (street address, if possible, with zip or postal code, and email address, if the author wishes) is provided in a footnote. Even for a single author, the full name of the person should be included. Follow the custom of individual countries regarding the placement of the zip or postal code. Note: For JAMA, this information is provided on the first page of the print article, not in the Acknowledgment section, and the heading used is “Corresponding Author.”

For smaller items (eg, letters to the editor, book reviews) with signature blocks rather than bylines, and where the signature block contains only the names and degree(s) of the author(s), a shortened form of address for the corresponding author may be used: Dr Jones, Mr Thomas, etc.

In JAMA, the signature block typically contains the author’s affiliation and email address:

To break an email address in print, always break before a punctuation mark so that it is clear that the address includes the mark.




This is especially important with hyphens and periods. There is no need to set the email address in italics or to precede it by the word email.
