A single index is the most convenient for the reader. However, separate author and subject indexes are common in biomedical publications, especially journals. Separate indexes should be “visually distinct”(p757) and be distinguished typographically and by running headers or footers.

AMA Manual of Style requires a subscription or purchase to access the full text of books within the service. Public users can however freely search the site and view the abstracts and keywords for each book and chapter.
